ALSS CONSULTATION
APPLICATION
PART A: CONSULTATION APPLICATION PROCESS:
1. The Application: A completed application is to be sent to the Executive Director of ALSS for consideration by the ALSS Board. (Retain a copy of Parts A, B and C for self.)
2. School Visit: If approved, the timetable of the consultation will be established. Allow up to a three-month period between the selection of the visiting team and the date of the consultation, to permit ample time for correspondence, exchange of information and planning.
3. Visit Schedule: The specific approach and schedule of the consultation will be determine by the host school, in communication with the consultation team.
4. Materials For Team Members: Depending upon the specific purposes of the consultation, following approval of an ALSS CONSULTATION, materials to be sent prior to the visit to the members of the consulting team should include:
a. Descriptive materials about the school: Brochures, handbooks, curriculum, tuition and fees, purpose statement, school history, enrollment breakdown, statistics, accreditation reports, and major changes experienced in the last years (enrollment, finances, and faculty).
b. Consultation materials: Faculty listings/profiles and assignments, master schedule, facilities description, course listings, and class sizes.
c. Finances: Budget, financial reports for the past several years, current balance sheet and financial reports for the year, indebtedness, projections, salary scales, and student grant-in-aid policies and summaries.
d. Structure: Articles of incorporation and bylaws, administrative structure, Board description and structure, description of the Association, Board manual, and description of relationships between the school and congregations and feeder schools.
e. Other: Any long range planning materials or projections, publications (yearbook/newspaper), and any other materials that help the visitors understand the issues to be addressed.
5. ALSS CONSULTATION Team Written Report: A final report will be developed as outlined in the consultation procedures accompanying this form. The final report, prepared by the consultation team leader, will be sent to the administrator and the chairperson of the Board of Directors of the visited school and the president of ALSS. Additional copies must be authorized by the administrator of the visited school, or if that position is vacant, by the Chairperson of the Board of the visited school. The visited school may wish to send copies to the District Education Executive and/or the School Service Department of synod.
6. The CONSULTATION Action Plan Report: The Action Plan Report must be submitted no later than three (3) months following the consultation to the ALSS Executive Director. Final reimbursement of expenses will be made to the host school following receipt of the report. Fifty percent (50%) of projected expenses are forwarded to the host school by ALSS prior to the team visit.

The purpose of the ALSS CONSULTATION is to provide a team of Lutheran educators who will offer encouragement, support, and assistance to a Lutheran High School that is facing special concerns.
PART C: NARRATIVE OUTLINING THE NEED AND GOALS
On a separate paper you are asked to provide complete answers to the following questions, numbering your response to correspond with the question. Please remember to answer ALL questions.
1. Why are you requesting a site visit? Please be specific in identifying the issues or questions that you want addressed.
2. Identify objectives you seek to meet through this consultation. Relate these to your response to question #1.
3. What specific expertise would you look for in the team of consultants?
4. What could your District Education Executive contribute to this consultation? (Note: You may invite him/her with the understanding that this will be at your District or your expense).
5. What activities, meetings, observations involving the consulting team would you expect to take place during the visit?
6. Tentative schedule: Please list dates/events and major activities of the consultation.
PART D: ESTIMATED BUDGET
The normal maximum expense is $1500 for a team of two. The projection below should total expenses for the team of two individuals.
